Your Visit with MABI
Useful information about your patient experience with MABI:
Patient Forms:
Please print these patient forms, fill out, and bring with you at your first evaluation with MABI. A member of our team will walk through the answers with you in person. Note: not all forms may be necessary for you to fill out, and the Balance Institute Coordinator can answer questions you may have.
Medications:
MABI team members may suggest that you discontinue a certain medication before your inner ear testing. The Balance Institute can only recommend that you discontinue a medication that may interfere with our testing, but your prescribing doctor should ultimately make this decision.
If this situation occurs with your medicines, MABI will instruct you on the specifics regarding discontinuing the medication.

Other Pointers About Your Visit:
- Please arrive 15 minutes prior to your appointment with completed paperwork. Patients arriving late may need to reschedule.
- MABI works cooperatively inside various SERC Physical Therapy clinics across Kansas City. You are a Balance Institute patient, but you are receiving your test inside of a SERC clinic.
Because of this, please inform the front desk that you are a balance patient so the staff can make your experience run smoothly.
- Wear comfortable clothing to each test.
- Bring your insurance card.
- Ladies: Please do not wear makeup to your inner ear testing. Because eye goggles are used to record eye movement, the audiologist will not record an accurate reading with makeup on the eyes.
